Medical Readiness Clinic (MRC)

Location 

The MRC is in BHC Makalapa, near Makalapa Gate on Joint Base Pearl Harbor-Hickam. The physical address is 1514 Makalapa Drive, Honolulu, HI 96818.

Operational and Overseas Screenings 

• Needed for individuals who will be going operational or to a deployable unit 
• Must be completed within 30 days of receiving orders 
• Must have hard copy orders in hand 
• For initial inquiry, please pick up screening package at the MRC front desk (near medical records) 
• Once paperwork is verified by the MRC team, you’ll be scheduled for a virtual appointment

Instructions for Completing Electronic PHA 

1. Complete the online electronic Periodic Health Assessment (ePHA) questionnaire  

• First time users will need to register a new account 
• If you’ve previously initiated, but not completed, an assessment, continue that assessment rather than initiating a new one 
• Work through all screens of the assessment. You’ll know your assessment is complete when you see a check mark in the circle above "Service Member" on your ePHA home screen. 
• Ensure you can be reached at the phone number you provide on the assessment; your PHA is completed by phone 
• Have your 10-digit DOD ID number AND your Unit Identification (UIC) or Reporting Unit Code (RUC) on hand 

2. Notify the MRC via e-mail at usn.jbphh.navhlthclinpearlhi.list.nhch-mrc@health.mil that you’ve completed your portion of the ePHA. Include the following information in your email: 

• Full Name 
• DOD ID number 
• Command/Unit 
• Contact number 
• Email Address 

3.  Once your email is received and PART I is complete via ePHA, you’ll be contacted via phone to schedule. 

NOTE: 

1. Operational/overseas screenings, PHAs, PDHAs, and PDHRAs are conducted by phone.
