Family Medicine

Due to operational commitments, the Primary Care Medical Homeport Clinics at BHC Makalapa and BHC Kaneohe Bay are currently accepting limited new enrollments for non-active duty beneficiaries. Ancillary services such as radiology, laboratory, and pharmacy will continue to be fully available at current capabilities for all eligible beneficiaries.

We are working with other Military Medical Facilities and Health New Federal Service (HNFS) to ensure the provision of safe, high-quality care for our value beneficiaries. We appreciate your understanding and remain committed to providing quality health care that is deserving of your trust.

Federal Holiday and Training Closures

Non-Birth Parent
If you are bringing a child for an appointment and are not the birth parent, you must present a medical power of attorney. This includes the step parent who is the sponsor of the child. Please see clinic leadership if you have any questions.
TRICARE Enrollment, PCM Change or Transferring PCMs
- Active duty are enrolled to the Military Treatment Facility (MTF) that services their unit of assignment.
- New active duty family members within 30 minutes of an MTF are assigned with sponsor to the servicing MTF. If they live more than 30 minutes from an MTF, Family members can enroll to an MTF or a network provider near their home.
- Retirees and their Family members can enroll in the DOFM clinics as long as there is capacity.
- Patients turning 65 years of age with MEDICARE Part A and Part B must use their TRICARE for life benefits and find a primary care provider in the network. You will remain eligible for space available care (Space A) in our specialty clinics, and may utilize our lab, radiology, pharmacy and emergency services.
- Dependent parents must have their Sponsor complete an application for care which is sent to the Dependent Parent Review Board. Limited requests are being approved.
- TRICARE Standard patients and beneficiaries enrolled to the network are not seen in the DOFM clinics.
Specialty Referrals
Womack Army Medical Center does not require our patients to activate their referrals after an appointment in one of our clinics.
Referrals are not required for Urgent Care visits. Our TRICARE Prime beneficiaries have an unlimited number of Urgent Care visits.

Please allow seven business days to receive a letter in the mail with the available specialty clinic and authorization number for all referrals.

If needed, as early as 48 hours after the appointment with your provider you can call (910) 907-2778 (APPT), option #2 to discuss your specialty consult with one of the staff members in the Specialty Referral Clinic. If you leave a voicemail we will return the call within 2-3 business days. Please do not leave multiple messages about the same issue.
Late Policy
As an appointment-based clinic, it is imperative that our customers and clinic staff establish a cooperative partnership to adhere to appointment times. All clinics in the Department of Family Medicine will make every effort to see our patients in compliance with TRICARE standards.

We strongly encourage patients to arrive 10 minutes prior to their scheduled appointment time. This allows for completion of the nursing intake and time for your appointment. Patients who arrive more than 10 minutes late may be asked to reschedule.

If you cannot make your scheduled appointment, please call the cancellation line and reschedule. This will aid in timely patient flow, help decrease patient wait times, and allow other patients to book appointments.
No Show Policy
In order to provide the highest accessibility to our patients, we request patients call to cancel appointments rather than "no show." Your unused, canceled appointment may be used for another patient to gain access to care. Units will be notified of beneficiaries who demonstrate a pattern of failing to cancel appointments in a timely manner.
ID Cards for Children
All children 10 years of age and older are required to have a federal Identification Card (ID) card. We will give you a one-time authorization form that you can use for your visit today. Once you have received this authorization you are required to show your child's ID card within 30 days.
